Asthma/COPD Management
If you have been advised by the surgery to submit an annual review of your asthma/COPD symptoms, please complete our Asthma Review form or our COPD Assessment form.
Diabetes Management
If you have been advised by the practice to complete a diabetes review, please complete our Diabetes Review form.
Insulin Starts
Smoking Cessation
If you have been advised by the surgery to submit smoking reviews on a regular basis, please complete our Smoking Review form.
Coronary Heart Disease Monitoring
If you have been advised by the surgery to complete an annual heart disease review, please complete our Coronary Heart Disease Review form.
Cervical Smears
The practice offers a cervical smear screening service, and all women between the ages of 25 and 60 will automatically be invited every three to five years to attend.
The test results are usually available after about 2 weeks.
We therefore urge all women between 25 and 65 who have not had a smear test in the last five years to ensure that they book one as soon as possible with the practice nurse.
